Nobody rolls Initiative.
That is important.
Horror in D&D does not begin when the monster attacks. It begins when the players realize something is wrong but do not yet understand exactly what it is.
Battle maps, lighting, sound, pacing, information, and encounter structure can all reinforce that feeling. The goal is not to make the characters helpless. It is to make the players temporarily uncertain about which choice is safe.
What actually makes D&D horror work?
Horror depends on uncertainty.
Players are usually very good at understanding normal D&D encounters. There is a room. There are enemies. Initiative begins. The party can see most of the battlefield and immediately starts calculating movement, attacks, spell ranges, and target priority.
That confidence is useful in heroic fantasy.
Horror asks you to disrupt it.

Treat light as information.
Under the current fifth-edition vision rules, Dim Light creates a Lightly Obscured area. Characters have Disadvantage on Wisdom (Perception) checks that rely on sight when trying to see something in a Lightly Obscured space.
Darkness creates a Heavily Obscured area, significantly limiting what characters can perceive without an appropriate sense or source of illumination.
That gives lighting direct mechanical relevance before you add any horror-specific rules.
Do not ask only whether the party can see. Ask how much of the location they can understand at once.
A torch is much more interesting when it reveals only one piece of a larger room.
The party sees the overturned dining table but not the doorway beyond it. They can see blood near the staircase but not where the trail ends. They can see a statue in the center of the chamber, but the walls behind it remain dark.
The room has not become mechanically complicated. It has become incomplete.
Do not forget Darkvision
Many characters can see in darkness through Darkvision or another special sense. Horror design should account for that instead of simply pretending those abilities do not exist.
Darkvision does not need to ruin a horror encounter. You can still use walls, doors, corners, heavy fog, elevation, architecture, concealed spaces, and limited range to control what characters know.
The goal is not to make every room black. The goal is to make complete information difficult to obtain.
What makes a good horror battle map?
A horror map should create uncertainty about where danger can come from.
Large empty rectangles are usually weak horror spaces because players can understand the entire tactical situation immediately.
Instead, use maps with:
Multiple connected rooms.
Corners that break sightlines.
Long hallways with side doors.
Windows into inaccessible spaces.
Stairs between floors.
Alternate routes.
Small rooms connected to larger chambers.
Hidden or concealed passages.
Areas visible before they are reachable.
The map should create questions before it provides answers.

On a physical tabletop, you can cover unexplored portions of the map, use removable paper sections, or display only the current area on a tabletop screen.
On a virtual tabletop, use fog of war, token vision, walls, doors, and lighting to control how much of the map players can see.
Roll20's Dynamic Lighting can use barriers, token vision, and Explorer Mode so players see the environment based on what their characters can currently perceive. Foundry VTT similarly combines walls, lighting, token vision, and fog exploration to restrict and reveal a scene.
That makes exploration itself part of the horror.
Opening a door is no longer just moving to another room. It reveals another piece of the unknown.

Why should the monster appear later than you think?
Once players can clearly see the monster, measure its distance, read its abilities, and attack it every round, part of the uncertainty disappears.
That does not mean you should hide the creature for the entire session.
It means the reveal should be earned.
Before the monster appears, reveal consequences.
A door has been ripped from its hinges.
Something has dragged a body into the ventilation shaft.
A survivor refuses to enter the basement.
The party hears something moving on the floor above them even though they were told the house is empty.
A large set of tracks begins in the snow and simply stops.
Show what the threat has done before you show exactly what the threat is.
Players begin constructing their own version of the monster in their heads. The eventual reveal now has something to compete with.
How do you pace a horror session?
Do not maintain maximum tension constantly.
Players become accustomed to any emotional state that lasts long enough. If every door screams, every hallway contains blood, every room has a monster, and every NPC speaks in a creepy whisper, the horror becomes normal.
Instead, alternate pressure and release.
Give players enough safety to lower their guard, then introduce a new piece of information that makes the situation worse.
A simple horror pacing sequence might look like this:

Give them rooms where nothing attacks.
Let them investigate.
Let them close a door.
Let them light a fire.
Let them find useful information.
Then let the next clue make that safety feel less certain.
Maybe the room has no second entrance, but footprints appear outside the door anyway.
Maybe the fireplace contains bones.
Maybe there is writing on the inside of the barricaded window.
Nothing attacks them.
That can be worse.
How should sound work with a horror map?
Use sound to give players information without giving them location certainty.
The party hears footsteps upstairs.
They hear a door close somewhere behind them.
They hear something scratching from the other side of a wall.
They hear a child crying from the room they just searched.
The sound tells players that something is happening. The map tells them all the places it might be happening.
This is another reason maps with branching corridors, multiple floors, adjoining rooms, and blocked sightlines work well for horror.
A sound in a simple square chamber has one likely origin.
A sound in an old manor with twelve rooms and two staircases creates a decision.
How should you use the Frightened condition?
Use Frightened when the game mechanics need to represent supernatural or overwhelming fear.
Under the current fifth-edition rules, a Frightened creature has Disadvantage on ability checks and attack rolls while the source of its fear is within line of sight. It also cannot willingly move closer to that source.
That is a significant tactical condition.
Do not use it as the only way to create horror.
A character can be mechanically unaffected by Frightened while the player is still deeply concerned about opening the basement door.
The Frightened condition is a mechanical effect. Horror is the atmosphere and decision-making pressure surrounding it. You do not need to impose Frightened every time you want a scene to feel scary.
How do you make a monster scary after combat begins?
Give the monster a relationship with the map.
A monster standing in the center of an illuminated room becomes a normal combat target very quickly.
A monster that moves through vents, climbs walls, extinguishes lights, opens doors from another room, drags characters around corners, or disappears into areas the party cannot see remains connected to the horror structure of the encounter.
You do not need to invent special rules for every creature.
Build the map around what the monster already does well.

A room where every mirror has been covered suggests that someone knew what they were afraid of.
These details are effective because players discover them rather than receiving a lore explanation before entering the location.

What usually ruins a horror D&D session?
The first problem is revealing too much too early.
If the party sees the entire dungeon, the monster token, every exit, and every tactical feature before the first decision, much of the uncertainty disappears.
The second problem is trying to scare players by removing every useful ability they possess.
Darkvision works. Divination works. Detection spells work. High Perception works. Let characters feel competent.
Then give them information that creates new questions.
The third problem is making every clue a false alarm.
If the scratching sound is always a rat, the players learn not to care about scratching sounds.
Sometimes the strange noise should be harmless. Sometimes it should be evidence. Sometimes it should be the monster.
The fourth problem is never allowing relief.
Constant tension eventually stops feeling tense.
The fifth problem is confusing horror with character helplessness.
Players should worry about making the wrong decision, not feel like their decisions do not matter.
